Laboratory Requests & Reports
Alberta Precision Laboratories looks forward to working with you to provide quality diagnostic services. Completing laboratory request forms with the required information is integral to ensuring prompt report delivery.
We recognize that the information requested can differ from requisition to requisition. Many requisition completion aids are provided to assist with requisition completion.
Report delivery options include electronic delivery to physician office systems (POSP/EMR) and paper delivery. Results are also available on Alberta Netcare, the provincial EMR.
New Provider Set-up
New providers to the province must complete and submit paperwork to be set up as a provider in each of the Information systems; Millennium/SCM, Meditech, Sunquest, and Connect Care.
Release of Results Directly to Patients
APL encourages patients to obtain their test results through consultation with the ordering physician.
Patient wishing to obtain their record must follow the access request process. There is a charge for providing patient lab records: Laboratory Results and Records.

Connect Care
The Province of Alberta is currently implementing a province wide CIS, Connect Care. Samples may be tested at more than one location throughout the province. Complete information on forms and requisitions if the best first step your office can take for timely laboratory results delivery.
