Biomarker (BRAF) Testing

On occasion, Oncologists may need to request BRAF testing on patients with Melanoma whose specimen has not been tested before.

Oncologists may submit a request using Referral of Pathology Material - Oncology APCY1006C.

Fax the properly completed form to CLS Consult Desk at Foothills Medical Centre, McCaig Tower at 403-944-4748.

It is imperative that the following information be provided and is accurate:

• Patient demographics - full name, DOB, Alberta healthcare number
• Requesting physician's full name and location
• Clinical diagnosis and stage of disease
• Type of specimen, pathology accession number if available

NOTE:

• Test requisitions MUST be completed by requesting physician or designate.
• Laboratory staff or pathologists will NOT accept phoned requests or other alternative methods.
• The requisition form is a legal document and is subject to audit by the College of Physicians & Surgeons of Alberta for accreditation.
• Alberta Health Services provide clear and strict guidelines for acceptance or rejection of incomplete requisitions to avoid erroneous results and risk of patient harm.
